@jeremystretch this is a specitifcation for the form factor MDC:

Additional, but not for my need, there is one more very small form factor - SN:
http://www.qsfp-dd.com/wp-content/uploads/2020/07/SN-Receptacle-Specification-Rev1.1.pdf

And, as usual with fibre, there are two different polishing methods, APC and UPC for each multimode and singlemode:
